The “4 hours” in this headline refers to an important treatment window for some ischemic strokes, but it does not mean you have four hours to wait. Stroke is an emergency, and treatment should begin immediately. CDC+1

If you suspect a stroke, do this immediately

  • Recognize B.E. F.A.S.T.: sudden Balance problems, Eye/vision changes, Facial drooping, Arm weakness/numbness, or Speech difficulty.
  • Call your local emergency number immediately. In the U.S., that’s 911; elsewhere, use your local emergency service.
  • Note the exact time the symptoms began (or the last time the person was known to be normal). This can determine which treatments are possible. CDC+1
  • Use an ambulance rather than driving yourself. Emergency personnel can begin care during transport and alert the hospital. CDC
  • Don’t give food, drink, aspirin, or other medication unless instructed by medical professionals. A stroke can be caused by either a clot or bleeding, so treatment must be determined after medical assessment and brain imaging.

Why the 4.5-hour window matters

For eligible ischemic-stroke patients, clot-busting medication such as alteplase or tenecteplase can be given within 4.5 hours of symptom onset. The earlier treatment is given, the better the potential outcome. www.stroke.org+1

Some patients with a large blocked artery can undergo mechanical thrombectomy, a procedure that physically removes the clot. In appropriately selected patients, this can be performed up to 24 hours after symptoms began, based on imaging and other criteria. www.stroke.org+1

Most important: don’t wait for symptoms to improve. Even if weakness, speech problems, or facial drooping disappear after a few minutes, it could be a TIA (“mini-stroke”), which still requires urgent medical evaluation. CDC
