10 Often-Overlooked Amlodipine Side Effects That Deserve Attention

Amlodipine is generally well tolerated, but it can cause a range of side effects. Ankle swelling is particularly important because it is relatively common and becomes more frequent at higher doses. DailyMed+1

  1. Ankle and leg swelling — Peripheral edema is the most characteristic side effect. In clinical trials, edema occurred in about 3% of people taking 5 mg and 10.8% taking 10 mg, compared with 0.6% taking placebo. DailyMed

  2. Dizziness or light-headedness — Amlodipine lowers blood pressure, and some people experience dizziness, particularly after starting treatment or increasing the dose. DailyMed

  3. Palpitations — Some people notice a pounding, racing, or unusually noticeable heartbeat. This was reported more often at higher doses in clinical trials. DailyMed

  4. Flushing or feeling unusually warm — Blood-vessel dilation can cause facial flushing or warmth. DailyMed

  5. Fatigue or unusual weakness — Fatigue was reported in clinical trials and can sometimes be overlooked as simply being tired. DailyMed

  6. Nausea or abdominal discomfort — Nausea and abdominal pain are documented adverse effects. DailyMed

  7. Constipation or diarrhea — Changes in bowel habits are among the gastrointestinal effects reported with amlodipine. DailyMed

  8. Muscle cramps or aches — Muscle cramps, muscle pain, and joint pain have been reported. DailyMed

  9. Gum enlargement — Gingival hyperplasia is an uncommon but recognized adverse effect. If your gums become enlarged, swollen, or bleed easily, mention it to your dentist and prescriber. DailyMed

  10. Sleep or nervous-system changes — Less common reported effects include insomnia, abnormal dreams, anxiety, tremor, tingling, and vertigo. DailyMed+1

🚨 When to seek help

Seek urgent medical attention for fainting, severe breathing difficulty, swelling of the face/lips/tongue, or severe chest pain. Rare cardiovascular and allergic reactions are listed in the prescribing information. DailyMed

Don’t stop amlodipine suddenly or change your dose without talking with your prescriber. If you’re experiencing troublesome ankle swelling, dizziness, or palpitations, a clinician can determine whether the medication, dose, or another condition is responsible.

If you’re taking amlodipine specifically because of swollen ankles, that is worth discussing with your doctor—the swelling can be a medication effect rather than simply “too much water.”
