Why Do These Small White Stones Appear in the Mouth and Smell So Bad?

Those small white or yellowish, foul-smelling stones are most likely tonsil stones (tonsilloliths).

What are tonsil stones?

They form when food particles, mucus, dead cells, and bacteria become trapped in the small grooves and pockets (crypts) of the tonsils. Over time, the material can harden or calcify into small white, cream, or yellow lumps.

The bad smell is mainly due to odor-producing bacteria and sulfur-containing compounds trapped in the material.

Why do some people get them?

You’re more likely to develop tonsil stones if you have:

  • Deep or prominent tonsil crypts
  • Frequent tonsillitis or throat infections
  • Post-nasal drip
  • Chronic nasal congestion
  • Dry mouth
  • Poor oral hygiene

Some people get them repeatedly simply because their tonsils have deeper crypts.

Are they dangerous?

Usually no. Small tonsil stones are generally harmless, although they can cause bad breath, an unpleasant taste, coughing, throat irritation, or the sensation that something is stuck in the throat.

Good oral hygiene—including brushing the tongue and cleaning between the teeth—can help. Gargling with warm salt water may also help loosen small stones.

Avoid aggressively digging them out with sharp objects, because you can injure the tonsil and cause bleeding or infection.

If you have large or recurrent stones, persistent bad breath, significant pain, difficulty swallowing, bleeding, or repeatedly inflamed tonsils, it’s worth seeing a dentist or ENT specialist.
